Referrals are welcome from individuals, families and carers, as well as professionals and organisations involved in a person’s support. This may include Support Coordinators, Plan Managers, allied health professionals, GPs, schools, community services, insurers and other service providers.

Can I make a referral for myself or a family member?

Yes. You’re welcome to make a referral for yourself or someone you support. If you’re unsure whether Social Bloom is the right fit, you’re also welcome to get in touch before submitting a referral.

Do I need to know which assessment I need?

No. You don’t need to know which assessment is most appropriate before getting in touch. We’ll consider the reason for referral, the person’s circumstances and the questions that need to be answered to help determine the most suitable assessment approach.

Absolutely. If you have questions about a potential referral, assessment options or whether Social Bloom may be the right fit, you’re welcome to get in touch first. There’s no need to have everything worked out before contacting us.

Once your referral is received, we’ll review the information provided and contact you to discuss the referral, clarify any additional information needed and confirm the next steps. Where appropriate, this will include discussing the assessment approach, appointments, fees and expected timeframes.

That’s okay. Circumstances and support needs can change. If something changes after a referral has been submitted, please let us know. We can discuss whether the assessment scope or approach needs to be adjusted and agree on the most appropriate next step.
