What services does Social Bloom Consulting provide?
Social Bloom Consulting provides comprehensive social work assessments and reports across NDIS, aged care, WorkCover, insurance, and private referral pathways. Services may include functional, psychosocial, mental health, capacity, housing, vocational, and support needs assessments tailored to individual circumstances and referral goals.
Who can make a referral?
Referrals may be made by individuals, families, support coordinators, healthcare providers, case managers, organisations, or other professionals.
How are services and assesments charged?
Fees vary depending on the assessment type, report requirements, funding pathway, and time involved. Information regarding fees, service agreements, and billing arrangements will be discussed prior to commencement of services.
How can assessments and reports help me?
Assessments and reports help identify an individual’s strengths, support needs, functional impacts, and barriers affecting daily life and wellbeing. Reports may assist with funding applications, support planning, service access, advocacy, rehabilitation, and care coordination.
Do you provide telehealth appointments?
Yes. Telehealth appointments may be available depending on the assessment type, individual circumstances, and suitability for remote service delivery. Where appropriate, appointments can be conducted via Zoom or other secure online platforms.

While Social Bloom Consulting is based in Cairns, services may also be available to individuals located outside of the Cairns region where telehealth is clinically appropriate and suitable for the assessment requirements.
What can I expect during my initial appointment?
The initial appointment involves discussing your background, current circumstances, support needs, goals, and the purpose of the assessment or referral. Information is gathered in a supportive, collaborative, and person-centred manner to inform recommendations and reporting outcomes
How many appointments or sessions will I need?
The number of appointments required will vary depending on the type of assessment, complexity of support needs, and reporting requirements. Some assessments may be completed in a single appointment, while others may require multiple sessions or collateral information gathering
